By writing my initials in the field above, I hereby testify that all of the consents, waivers, releases, and authorizations within have been determined by me and serve as true and accurate statements by me. Even the gentlest of grooming can cause stress to an animal. While necessary to the animal’s continued good health, it is an interaction outside the purview of its natural environment. It is always the intention of The Happy Cat Hotel to cease and desist upon the first indications of stress-related reactions. There are many techniques and approaches that Happy Cat Hotel Groomers are trained to implement for the purpose of limiting stress, but no approach is foolproof. While every cat is observed throughout the grooming process, there is no guarantee that a cat will not have a stress-related event. The Happy Cat Hotel reserves the right to decline service to any cat for any reason. Thus, I hereby agree to the foregoing as owner of the pet: 1. Dematting/detangling of matted hair may cause patchiness and hair loss. Consent to remove the matted hair is on this understanding. Shaving your cat will dramatically change the pet’s appearance, and the hair will be very close to the skin. This may expose pre-existing skin conditions for which The Happy Cat Hotel shall not be held liable. Removal of mats whether by lion cut (main body shave) or dematting procedure can be stressful to your pet and may exacerbate stress-sensitive health problems. Matting and/or pelting will result in additional charges (T&M) upon consultation, based upon the severity of the problem and the time required to correct it. I hereby agree to a lion cut and all applicable charges should Happy Cat Hotel's Executive Groomer deem this necessary for the health, safety, comfort, and humane treatment of my cat. Fecal removal, aggressive cats, and flea baths will all be performed as T&M services. 2. Certain illnesses and conditions including (but not limited to) diabetes, heart murmur, hypertrophic cardiomyopathy, seizures, and being overweight result in a higher risk of incident during grooming and I hereby agree to sign an "At Risk Pet Form" prior to my cat's appointment. 3. Previously discussed grooming standards (i.e lion cut, comb cut, etc.) may not be completed due to my cat's health condition. This decision is in the cat's best interest to prevent harm from coming to them. The client may be responsible for payment of any services that are completed in lieu of or in addition to the previously discussed styles. 4. In the event of any injury or health concern, Happy Cat is authorized to contact a vet and authorize any veterinary treatment for the cat as necessary. The owner is responsible for all veterinary charges that may incur as a result. The Happy Cat Hotel will attempt to contact you in the event of an emergency. Cats at a higher risk of injury or health problems than the average pet tend to be cats that are: matted/pelted, elderly, those with pre-existing health conditions. Any grooming that takes place on a high risk cat will be performed at the owner's risk. Elderly cats are at a higher risk for nicks and cuts from shaving. Grooming may expose pre-existing health and/or skin problems for which The Happy Cat Hotel cannot be held liable.
