Emergency Care Authorization

This form is to be updated every 2 years or with any new feline companion added to the profile / reservation. Please contact the front desk to make changes outside of annual update.

Emergency

In the event that I or my emergency contact and/or preferred veterinarian cannot be reached;

I grant THE HAPPY CAT HOTEL full authorization to make any decisions concerning the care and well being of any of my cat(s) boarding at THE HAPPY CAT HOTEL now or as part of any future reservation.

Should a medical condition or other emergency arise for any reason, I authorize the THE HAPPY CAT HOTEL to facilitate and expedite any necessary transport of my cat, treatment by any HAPPY CAT HOTEL staff as necessary including First Aid/CPR, and treatment by the nearest 24hr VETERINARIAN HOSPITAL or licensed VETERINARIAN. At the point of veterinary intake, all medical decisions will be made by the clinic/hospital, without exceptions.

I authorize THE HAPPY CAT HOTEL to apply any costs incurred as a result of emergency treatment to the card on file with my reservation or made payable by me directly to THE HAPPY CAT HOTEL as part of my reservation charges as reimbursement should I not be available to arrange payment directly with the veterinarian facility of action.
